Information Provided at Registration

HealthAtoZ pledges to protect the privacy of all of our registered users. The HealthAtoZ registration form requires users to give us contact information (name and e-mail address), and demographic information (ZIP code or age). To use some site functions, users must volunteer personal health information (such as, height, weight and past medical history). There is no financial information collected.

We use contact information from the registration form for internal purposes, to send the user news, information about new HealthAtoZ site features, services or updates, information about our company. You will be given the opportunity to decline these services at the point of registration, and you may opt-out of receiving future mailings (see the Choice/Opt-out section following).

Demographic and personal health information is used to tailor the visitors' experience at our site, showing them content that we think they might be interested in, and displaying the content according to their preferences. Your health information is password-protected to prevent access by other users.

HealthAtoZ will not share, sell or forward to third parties any individually identifiable personal information (e.g., your e-mail address, name, etc.) that you supply during the registration process; this includes health insurers. You can be assured that our content providers will not have access to your personal information. HealthAtoZ may, however, provide your personal health information and that of other registered users in aggregate form to third parties, but never in such a way that it violates your privacy. This means that we will never provide anyone's individual health records to any third party, although we may provide advertisers with information about the prevalence of certain health conditions in the overall group of HealthAtoZ subscribers, and similar aggregate information.

HealthAtoZ does not knowingly collect personal information from any person under the age of 13. We are committed to protecting the privacy of children, and this Web site is not designed or intended for children under the age of 13.

All personal, identifiable information is retained by Medical Network, Inc. (d.b.a. HealthAtoZ.com) for a period of five years/indefinitely, unless a member requests that the information be deleted. The Health Risk Appraisal employed by HealthAtoZ for personalization purposes is hosted on the University of Michigan Health Management Research Center's site. Information passed between the two sites is encrypted and secure. All information is kept confidential and private. As an alternative to taking the HRA, users can choose to select their topics of interest.

Online Surveys

We may run surveys on our site in which we ask visitors for contact information (such as their e-mail addresses). We use contact data from our surveys to send users information about our company. HealthAtoZ E-Health Dashboard, Health Managers and Personal Health Record

As part of registration for the HealthAtoZ E-Health Dashboard, Health Managers and Personal Health Record you will be requested to provide contact information, demographic information, and personal health information and to take an online Health Risk Appraisal to enable you to use these programs fully. Please see the HealthAtoZ Terms of Service for more details on the type of information gathered, how it is used, and how your privacy is protected.

Users may opt to select their health topics of interest manually rather than complete the Health Risk Appraisal.

Public Forums

HealthAtoZ makes chat rooms, forums, message boards and/or news groups available to our users. Please remember that any information disclosed in these areas becomes public information, and you should exercise caution when considering the disclosure of your personal information. HealthAtoZ is not responsible for material posted.

We provide a live on-line nurse chat feature (24 hours per, seven days a week) on our website. When used, this feature does not collect any personally identifiable information. We use the information provided to communicate with you regarding your inquiry.

This site has security measures in place to protect the loss, misuse and alteration of information under our control. Information provided to the various sections of the HealthAtoZ site is password-protected, encrypted and protected by means of a firewall.

HealthAtoZ.com is committed to privacy protection and utilizes the most current security tools available to ensure the privacy and confidentiality of all registered users. An individual user ID and password protect each HealthAtoZ account so that no one other than the user can access its features. In addition, all information is protected through secure socket layer (SSL) software, which encrypts the data to protect it during transmission. HealthAtoZ.com maintains this information, encrypted, on a secure, protected server. HealthAtoZ's E-Health Dashboard, Health Managers and Personal Health Record are certified by VeriSign (http://www.verisign.com) as secure health information tools.

Management of PII and PHI

As noted in the Privacy and Security section of this policy document, HAZ is committed to protecting the Personal Identification Information (PII) and Personal Health Information (PHI) provided to us. Processes will be implemented and constantly maintained to ensure such information is protected utilizing the highest industry standards.

Warnings will be made whenever a user is entering such information into unprotected portions of the Web environment.

We reserve the right to disclose your personally identifiable information as required by law and when we believe that disclosure is necessary to protect our rights and/or to comply with a judicial proceeding, court order, or legal process served on our Web site.

Choice/Opt-Out

HealthAtoZ provides users the opportunity to opt-out of receiving communications from our partners and us.

When you complete registration or when you make changes to your profile (Edit Profile), you will be given the choice of receiving informational/promotional e-mails from HealthAtoZ. You may opt-out of receiving e-mails on behalf of HealthAtoZ at registration by removing the automatic check mark from the preference box for receiving e-mails and newsletters. You may also opt-out of receiving these e-mails and newsletters by changing your preferences in Edit Profile. You may remove all of the information you've stored with us through the "Edit Profile" screen. If you wish to remove your information, the process takes three business days and you will be notified via e-mail upon completion of the removal process.

If you choose to remove or not supply us with personal information, you will be unable to access some of HealthAtoZ.com's features. You will be unable to access personalized features such as trackers, the E-Health Dashboard, Health Managers, the Personal Health Record, personalized and dynamic content and e-newsletters. You will still have access to our static content, risk assessments and un-personalized tools.

Notification of Changes

If we are going to use your personally-identifiable information in a manner different from that stated at the time of collection, we will notify you via e-mail.
