The following text discloses our information-gathering practices with respect to HealthAtoZ E-Health Dashboard, Health Managers and Personal Health Record users. Type of Information Gathered
The HealthAtoZ registration form requires users to give us contact information (name and e-mail address), personal health information (gender, date of birth, and additional optional health information), and demographic information (ZIP or postal code). Additional health information can be stored on our site by the user but is not necessary to use the site. This additional information gives the user access to personalized information, advanced health management programs and personal health records.
How the Information is Used
To use some site functions users must volunteer personal health information (e.g., height, weight, and past medical history). There is no financial information collected.
HealthAtoZ will not sell or forward to third parties any individually identifiable personal information, e.g., your e-mail address, name, etc., that you supply during the registration process; this includes health insurers. You can be assured that our content providers will not have access to your personal information. HealthAtoZ may, however, provide your personal health information and that of other registered users in aggregate form to third parties, but never in such a way that it violates your privacy. This means that we will never provide anyone's individual health records to any third party, although we may provide advertisers with information about the prevalence of certain health conditions in the overall group of HealthAtoZ subscribers, and similar aggregate information.

The Health Risk Appraisal employed by HealthAtoZ for personalization purposes is hosted on the University of Michigan Health Management Research Center's site. Information passed between the two sites is encrypted and secure. All information is kept confidential and private.
As an alternative to taking the HRA, users can choose to select their own topics of interest.

HealthAtoZ's E-Health Dashboard, Health Managers and Personal Health Record are certified by VeriSign as secure health information tools.
Children
HealthAtoZ does not knowingly collect personal information from any person under the age of 13. We are committed to protecting the privacy of children, and this website is not designed or intended for children under the age of 13.
Choice/Opt-Out
The HealthAtoZ provides users the opportunity to opt out of receiving communications from us and our corporate partners at the point where we request the user to register for use of our services.
HealthAtoZ gives users the following options for removing their information from our database, to not receive future communications or to no longer receive our service. You can use the "Edit Profile" feature to remove Profiles you have created for family members, and to delete information from your own Profile.

User Complaint Policy
HealthAtoZ from time to time receives complaints. To better serve our users, we respond to complaints that address the site or raise a legitimate concern or problem in a timely manner - generally, within three business days.
Any items that can not be completely addressed through standard processes are to be escalated to the QOC for resolution.
Summary information regarding complaints is compiled and reported to the QOC on a monthly basis.
Your Responsibilities as a HealthAtoZ User
Obtaining and maintaining the equipment necessary to access and to use HealthAtoZ's services (computer hardware, telephone line, etc.) is your responsibility. You also are responsible for maintaining the confidentiality of your user identification and password. HealthAtoZ does not accept "advertorials". Advertorials are articles provided by sponsors with the sole purpose of promoting a product or service. However, advertisements on HealthAtoZ.com may link to off-site advertorials over which HealthAtoZ has no control.
Advertising and Collected Health Information
HealthAtoZ occasionally shares anonymous aggregate statistical information regarding site usage and traffic with advertisers and sponsors. This information cannot be linked to individual users and is entirely anonymous.
Advertisements are not directed to individual users, but may be targeted to individual conditions and their relevant sections on the site. We do not "push" advertisements to users based on stored information.
Policy Review
Medical Network Inc. reviews all HealthAtoZ.com policies annually. If MNI determines a change in policy is needed, the Quality Oversight Committee drafts a revision of said policy, and issues it to the Board of Directors. If approved by the Board of Directors, the policies are sent to the affected parties 30 days in advance of it going into effect. Said parties then have 30 days to review the changes and submit comment. After the 30 days, the policy will go into effect or be re-evaluated by the Board of Directors and MNI. As part of the review process, the master electronic copy of all policies is referenced with the master hard copy signed by the officers of MNI and HealthAtoZ and kept in the HealthAtoZ archives.
Medical Network Inc. maintains both hard and electronic copies of all policies at the office of HealthAtoZ.com. All HealthAtoZ staff members are issued a hard copy and can request additional copies.
Ethics Codes
HealthAtoZ subscribes to the Trust-e privacy code. Verification of HealthAtoZ's compliance can be found at: www.truste.org
HealthAtoZ's Quality Oversight Committee considers and applies professional guidelines, principles and ethical codes to their strictest letter. When the Quality Oversight Committee meets, agenda items that relate to guidelines, principles and ethical codes are discussed based upon the Trust-e privacy code. The QOC evaluates the agenda items, debating the appropriate actions to take based upon the codes adopted. If the action item is a specific task, a committee member is assigned to oversee the action. The QOC is then briefed at every meeting until the task is completed.
If the action requires a policy change or adoption of additional professional guidelines, principles and ethical codes, the Quality Oversight Committee develops them according to adopted professional guidelines, principles and ethical codes based on the following criteria:
- Is the policy, professional guideline, principle and/or ethical code compliant with both letter and spirit of existing professional guidelines, principles and ethical codes?
- Do the proposed professional guidelines, principles and ethical codes enhance, strengthen or build upon the effectiveness of previously adopted professional guidelines, principles and ethical codes?
Delegation
All contracts with third parties providing service on behalf of HAZ will clearly:
- Define the responsibilities of HAZ and the contractor
- Define mechanisms to address any problems with the contractor's performance
- Ensure compliance with the same standards employed by HAZ
HAZ will implement processes to maintain oversight of the activities/services delegated to the third parties.
Accessibility
Accessibility for the Disabled
HealthAtoZ makes a diligent effort to make HealthAtoZ.com compatible with standard accessibility options for the disabled. This includes, but is not limited to, alternate font sizes and programs that aid the visually impaired. However, this compatibility is not made at a cost of functionality or experience to users without special needs.
Protection of Children
HealthAtoZ does not knowingly collect personal information from any person under the age of 13. We are committed to protecting the privacy of children, and our Web site is not designed or intended for children under the age of 13.
Business Relationships
Relationships with businesses or individuals will be established only with those that are committed to similar standards and ethical practices. Potential conflicts of interest may be reason for not committing to or discontinuing a relationship. In all established relationships, such conflicts will be fully disclosed.
Consummated relationships must also meet all the contractual and ethical standards as well as other standards identified elsewhere in this policy document.
Response to Users for Inappropriate Use
As noted in our Disclaimer, "HealthAtoZ site visitors are prohibited from posting or transmitting any unlawful, threatening, libelous, defamatory, obscene, inflammatory, pornographic, criminal or profane material or any material that could constitute or encourage conduct that would be considered a criminal offense, give rise to civil liability or otherwise violate the law."
When such inappropriate use is identified, the user will be informed of the offense and notified that such further usage could result in the termination of his or her access to the service. In instances in which termination is required, notification will be sent to the user by HAZ.
Roles and Responsibilities for Reporting to External Authorities
Except as dictated by state or federal laws, a designee of the MNI Board of Directors or a representative of the Quality Oversight Committee will be provide all reporting to external authorities.
